DescriptionPosition Summary:
The Specialized Services Associate works with individuals with developmental disabilities who live in nursing facilities. Associates provide individualized training and support for individuals to pursue personal goals, learn new skills and participate in community activities. Specialized Services Associate is a position under the Supervision of PASRR Specialized Services Coordinator.
DUTIES and RESONSIBILITES:
-Assure that individual’s rights are protected and that they are free from abuse or maltreatment.
-To encourage each individual to offer his or her unique gifts and talents to the community. Promote meaningful community involvement and social growth. Encourage each person to grow and develop according to his or her own capacities. Assist with forgoing new friendships, visiting community programs, encouraging and assisting in the normalization of daily activities.
-Provide services according to the PCSP, including Frequency and intensity specified. Monitor progress toward outcomes.
-Attend quarterly care plan meetings and annual PCSP update meetings.
-Collaborate with individuals, their family or guardian, and other team members.
-Maintain all appropriate documentation in a professional and timely manner.
-Report and document all incidents according to LifeSkills P&P and DDID regulations.
-Attend all required trainings and update training classes and any additional other duties as assigned by supervisor.
JOB REQUIREMENTS/ EXPECTATIONS:
Physical demands of a Specialized Services Associate is the ability to lift up to 50lbs if needed. Specialized Services Associate will be in and out of Nursing Facility providing services, the job requires being able to have visual requirements which include being able to see up close and far away, peripheral vision and ability to adjust focus. Position also requirements the ability to stand, walk, use hands (fine motor skills), stoop, kneel, crouch and talk or hear. The position can requirement transporting clients in the community. The position does require traveling to other counties within our 10 county district, must have reliable transportation, valid driver's license and current automobile insurance.
Specialized Service Associate position in under the supervision of PASRR Specialized Services Coordinator. Position requires annual employee evaluation conducted by supervision. Monthly review of all Specialized Services Associate's documentation of services being provided.
QualificationsJOB QUALIFICATIONS:
Position requires a high school diploma or GED.
Minimum of one-year experience working with individuals with intellectual and developmental disabilities is preferred.
Valid driver’s license and current automobile insurance.
