Looking for Social Worker for Independent Living (Life Plan Community) in Washington DC
Salary $71,510K to $89,377K commensurate with Experience
Job Requirements:
Bachelor's degree in social work from an accredited university. One (1) to three (3) years’ experience in a Senior Living community required, or equivalent combination of education and experience. Knowledge of practices in Life Plan (Continuing Care) Communities. Compassion for older adults, and fluency with a person-centered approach which exhibits a commitment to hospitality and excellence in personalized assistance.
Resident & Family Interactions (or Scope of Practice)
- Conducts annual Wellness Visits with all Independent Living residents to ensure demographic and biographical information on is accurate.
- During Wellness visits, assesses resident engagement and overall well-being utilizing a Dimensions of Wellbeing model.
- Meets with residents and/or family members to discuss in-home care needs and facilitate communication related to transitions to higher levels of care when appropriate.
- Facilitate resident-centered psychosocial support groups.
- Lead weekly team discussions during Resident Review meetings, providing up-to-date information on resident care needs throughout the continuum of care.
- Lead and/or facilitate community forums that provide educational topics related to support infrastructure in our Life Plan community.
- Actively participate in Independent Living Resident Services team initiatives, activity planning, etc.
- Provide leadership connections to relevant resident-led monthly committees.
- Provides communication support to the Resident Services team utilizing the online resident portal and other community-based communication channels.
Grievance Responsibilities
Ensure that residents know how to file grievances, implement and manage the written grievance policy to ensure the prompt resolution of grievances. Ensuring and leading prompt response to a grievance and taking required actions.
• Ensures grievance policy meets requirements and regulations and is being followed.
• Educating residents on grievance policy.
• Ensuring a review of the policy and plan on an annual basis.

• Provides assistance to residents, visitors, or employees in an emergency situation; following the proper emergency procedures.
