Job Posting: Social Care Housing Navigator
Location: 324 w. buffalo st, Ithaca, ny 14850
Employment Type: Full Time 35hrs, M-F 8:30am-4pm
Salary: $24.00 an hour
General Description
The Social Care Network is a group of community-based organizations, healthcare providers, and other partners who are working together to provide screening, navigation, and delivery of Enhanced Health-Related Social Needs Services (HRSN) to Medicaid Managed Care Members. HRSN services include housing support, nutrition, and non-medical transportation. Catholic Charities Tompkins/Tioga is one of the organizations that is participating in the network.
Catholic Charities’ role as an Enhanced HRSN Provider is to provide Housing Support Services to Medicaid Members that have an identified housing social care need in the Tompkins County area. This position will focus on providing Pre-tenancy Services, Community Transitional Services, Tenancy Sustaining Services and Housing, Transition, and Navigation Services.
Essential Duties and Responsibilities
- Receives and responds to referrals of Medicaid members experiencing housing instability. Completes standardized screenings and eligibility assessments for Enhanced HRSN Housing Services.
- Provides Housing Transition and Navigation, Pre-Tenancy, Community Transitional, and Tenancy Sustaining Services to qualified Medicaid members.
- Becomes familiar with the Social Care Network program and the available services. Gains an understanding of the local and regional service providers that are participating in the Social Care Network, especially those serving Tompkins County.
- Links and refers program participants to local and regional service providers as appropriate.
- Creates and maintains Member Care Plans for individuals supported by the Social Care Network.
- Encourages and supports participant engagement with complementary services that promote housing stability and overall well-being (e.g., benefits, behavioral health supports, employment services).
- Works collaboratively with Catholic Charities staff, Care Compass, healthcare partners, community providers, landlords, housing providers, and utility companies to support continuity of care.
- Provides feedback to improve service workflows, referral processes, and identify community resource gaps.
- Maintains accurate and timely documentation and service data in required systems.
- Tracks service delivery, time allocation, and program activities to support HRSN billing, invoicing, and compliance
- Attends required meetings, supervision, and trainings related to HRSN services, Unite Us, and housing navigation best practices.
- Completes other duties as assigned.

Qualifications
Education: High School diploma or equivalent required. Associate’s degree or training in human services, social work, or related field preferred.
Experience:
- Experience working with individuals or families navigating housing instability, homelessness, or other social service systems or comparable experience in human services, case management, outreach, or service navigation.
- Willingness to learn and adhere to program requirements related to the Social Care Network, Medicaid 1115 Waiver, and HRSN services.
- Familiarity with housing resources, landlords, or housing-related services in Tompkins County or the surrounding region preferred.
- Experience using trauma-informed, client-centered, or harm reduction approaches preferred.
Relevant years of experience and education will be considered

- Strong interpersonal and communication skills, with the ability to build rapport with people in crisis using a respectful, nonjudgmental approach.
- Ability to conduct screenings, gather information, and complete required documentation accurately and on time.
- Ability to manage competing priorities and follow program guidelines in a fast-paced environment.
- Capacity to work both independently and collaboratively as part of a team.
- Ability to work in the following software programs: Windows, Microsoft Office, and multiple online software platforms as required by funders and partner organizations
- Ability to maintain confidentiality and handle confidential information with discretion
- Ability to demonstrate behaviors and attitudes which support organizational mission, philosophy, and policies
- Ability to work in a cooperative and helpful manner with all individuals
- Willingness and ability to foster agency and program wide cooperation and teamwork through use of positive/constructive communication techniques
