$74,547-$80,510 per year DOE.
The Spiritual Care Director provides and coordinates worship services, pastoral counseling, and spiritual support for residents, families, and staff in alignment with the community’s mission and values. This role promotes holistic care and supports psychosocial well-being in Assisted Living (RCFE), and, if applicable, Skilled Nursing Facility (SNF) and Independent Living settings.
Our Spiritual Care Director:
- Plans and leads worship services, religious observances, memorials, and end-of-life rituals.
- Provides individual and group pastoral counseling to residents and families, addressing grief, loss, life transitions, and spiritual needs.
- Partners with Life Enrichment and other departments to incorporate religious and spiritual events into the community calendar.
- Collaborates with external faith leaders to support diverse spiritual needs (e.g., rabbis, priests, ministers).
- Supports residents receiving palliative or hospice care and coordinate with hospice providers.
- If applicable, participate in interdisciplinary care planning and document pastoral visits in resident charts per SNF standards and Title 22 requirements.
- Educates and assists residents and families in understanding and completing Advance Directives.
- Provides support during resident transitions, including move-ins, hospital discharges, and end-of-life care.
- Maintains confidentiality and uphold residents’ dignity and rights in accordance with California regulations.
- Promotes relationships with external faith communities and religious organizations.
- Recruits, trains, and supervises pastoral care volunteers or chaplains, if needed.
- Participates as a member of the community leadership team.
- Offers spiritual education opportunities on topics such as ethics, grief, or interfaith understanding.
- Assists Life Enrichment teams with resident engagement and activities, when appropriate.
- Maintains knowledge of and compliance with applicable RCFE and, if applicable, SNF regulations related to pastoral and psychosocial care.
- Promotes the Mission, Vision and Values of Transforming Age in all areas of responsibility, and demonstrate commitment to Culture of Excellence and strategic goals.
