Applicants must live in or around Washinton County and be willing to drive throughout the county and meet with individuals face-to-face to complete Supports Coordination activities.
Supports Coordinator Minimum Requirements

Bachelor’s degree with successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social services.

OR at least12 college credits in fields listed above AND TWO YEARS’ professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work or health and rehabilitative services
Supports Coordinator Responsibilities

Coordinate community living supports that may include but is not limited to residential, vocational, employment, educational, and social organizations.

Respond to incidents to ensure appropriate actions are taken to protect the well-being, health, safety, and rights of the individual.
Benefits
Pay: $42,000 - $43,000

Education
|
Bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or other related social services.
OR
Two years’ experience as a County Social Service Aide 3 and two years of college level coursework, which includes at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social services.
OR
Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social services AND one-year experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions.

One-year experience working with individuals with disabilities.

Licenses, Certificates or Clearances
|
Act 33/34 and FBI Clearances.
Valid Driver’s License.
Car Insurance Coverage Verification.

Special Skills, Knowledge and Abilities

Computer competency with ability to learn agency software.

Knowledge of Community Resources.
