Position Summary:
The Life Skills Worker will assist the HOPE Program Team by providing hands-on assistance and support to HOPE Program (Permanent Supported Housing) participants, teaching and demonstrating a variety of daily life skills, including personal hygiene, budgeting, utilizing community resources to help augment basic needs, meal preparation, and maintaining a safe and clean home.
Essential Functions:
- Work in a team capacity with the HOPE Program team (HOPE Program Manager; Housing Specialist) and HOPE participants to identify and establish service plan goals in the areas of daily life skills.
- Meet with HOPE participants in the community and home setting to provide direct assistance, education, and support to help participants learn and implement identified life skills goals.
- Provide ongoing verbal and written communication on participants’ progress on their life skills goals.
- Documentation skills, including maintaining up-to-date service plan goals, and timely entry of all activities with participants in agency database system.
- Provide follow up to identified consumers.
- Organize and assist housing program participants with moving during program enrollment and discharge/termination.
- Maintain utilization requirements.
- Proficient computer skills.
- Other duties as assigned to support the mission of the agency.
Mostly Daylight, Monday-Friday hours, need to be flexible. No Weekends or Holidays. Travel required to local communities. Mileage reimbursement is available at the state set rate!
The hourly starting rate is $15.
Qualifications:
High School diploma or GED required. Associates degree in social service or related field preferred. At least one year experience working with adults with a serious mental illness and/or co-occurring mental health/substance abuse disorders.
Valid driver’s license, proof of auto insurance, Act 33/34 and FBI clearances required. Individuals who have accessed human services are encouraged to apply.
