Position Overview
As Social Services Director, you will plan, organize, and oversee all social service functions for residents and families while serving as a key member of the interdisciplinary care team. You’ll advocate for residents, guide families, coordinate admissions and discharges, and ensure compliance with state and federal regulations.
Key Responsibilities
- Provide ongoing resident and family counseling, including crisis intervention and adjustment support.
- Complete psychosocial assessments, quarterly reviews and MDS as required.
- Coordinate discharge planning, follow-ups, and post-discharge calls within 48 hours
- Assist residents and families with placement options and transitions
- Participate in care plan conferences, resident council meetings, and interdisciplinary team meetings
- Oversee admissions support, facility tours, and marketing/networking efforts
- Manage PASRR, MA-51s, 408s, MDS sections, and required documentation
- Address resident and family concerns promptly and professionally
- Lead and oversee social services operations
- Maintain department policies, procedures, and compliance standards
- Directly supervises assigned personnel for dept.
Qualifications
- MSW (Master of Social Work) Degree (strongly- preferred) or Bachelor’s degree in Social Work or equivalent experience
- Prior experience in short-term or long-term care strongly preferred
- Knowledge of admissions, discharge planning, and regulatory requirements
- Strong communication, organizational, and interpersonal skills

- Full-Time
- Monday – Friday | 8:00 AM – 4:30 PM
