We are seeking a Youth Family Foundations Case Manager to join CTI�s Youth Services team, located at the Youth Opportunity Center. The Youth Family Foundations Case Manager will provide advanced levels of support to young adults (14-24) and their growing families entering Youth Family Foundations programming. This includes providing advanced level case management, advocacy, referral information and supporting participants in overcoming barriers towards long-term, independent housing.
Responsibilities Include:
- Assess for general need for each family or individual.
- Connect to community supports geared towards family development.
- Increase access to nutritional supports.
- Connection to prenatal supports and in-home supportive services
- Increasing access to education and employment
- Creation of individualized goals with a youth-focused lens
- Reduce barriers and overcome obstacles towards stability
- Data tracking, entry, and submission into local and state systems.
- Support existing Youth Service programming and team members as needed.
Qualifications:
- Bachelor�s Degree preferred, preferably in a Human Services related field.
- Two years of experience working with youth and young adults preferred
- Ability to work comfortably with pregnant and parenting youth in both group and individual settings, preferably with experience advocating for families.
- One-year assessment, advocacy and case management work in a Human Service field preferred.
- Demonstrated knowledge of community resources available to low-income individuals and families.
- Excellent customer service, organizational, oral and written communication skills.
- Ability to manage multiple priorities and work in a fast-paced environment.
- Excellent crisis management skills.
- Ability to maintain accurate records and confidentiality of information.
- Excellent attention to detail.
- Valid Driver�s License
