Job Summary
A Community Living Support (CLS) professional provides one-on-one, in-home or community-based support to individuals with disabilities or special needs, aiming to increase their independence, safety, and community integration. Key duties include assisting with daily living tasks, such as cleaning, cooking, hygiene, and transportation, while following a personalized support plan. Age group 3 yrs to adult.
Key Responsibilities and Duties
- Skill Development: Develop Participant in, or assist with, activities of daily living (ADLs), including personal hygiene, cooking, laundry, and household maintenance.
- Community Integration: Assist individuals in accessing the community, including transportation to social events, recreation, and medical appointments.
- Documentation: Maintain accurate, timely, and legible records, including daily session notes, progress reports, and behavior tracking data.
- Personalized Care: Implement goals outlined in the consumer's Person-Centered Plan (PCP) to promote independence.
- Safety and Health: Monitor health, remind or assist with medication, and ensure a safe, clean environment.
- Mentorship: Offer encouragement and help develop social skills and relationships outside of staff. Education/Experience
- High school diploma or GED is required.
- License: Valid driver's license and a reliable vehicle with insurance, and a good driving record. Must be able to obtain a valid chauffeur's license.
- Skills: Strong communication skills, patience, and ability to work with individuals with diverse needs.
Knowledge, Skills, and Abilities:
Experience working with individuals with disabilities is helpful. The incumbent must: Obtain First Aid/CPR certification within 30 days of hire; training is provided in-house.
- Have an acceptable driving record.
- Have or be able to obtain a valid chauffeur's license.
- Have a cell phone.
- Setting: In-home and community-based, requiring travel between locations in Lenawee County using Goodwill vans.
Flexibility: This position is part-time for 20 hours weekly, requiring evenings and weekends.
Physical Demands and Work Environment:
- Must be able to bend, stretch, reach, and lift up to 40 pounds.
