Community Relations Manager- FSP, Pathways
Family Support Program (FSP) Coordinator:
To provide assistance to parents of severely emotionally disturbed children and adolescents on applying for the FSP program. Additionally, to intervene with an array of intensive community-based services in an effort to create a context for positive change.
The FSP coordinator will be based at one of C4’s sites, with about 80% of the services are provided in Chicago communities on the north and west sides and northern suburbs. Work may be stressful due to consumer behaviors, periods of high call volume, or tight deadlines.
Pathways to Success Community Relations Manager:
A key goal of Pathways to Success is to ensure that youth and families are connected to local community resources allowing them to maintain stability during their enrollment and after their transition from Pathways to Success. To further support this goal, CCSOs will be required to have an identified Community Relations Manager to identify and collaborate with existing local community networks throughout the CCSO’s DSA and in surrounding DSAs.
The Manager will ensure that care coordinators understand how to access community networks and that the information regarding the networks is included in the CCSO Community Resource Directory, as required in CCSO Handbook Section 214.13.
The Community Relations Manager will be provided with a list of existing local community networks from HFS with the expectation that the Manager connect with each of the networks to gain understanding of the services and/or supports that the networks provide, individuals typically served and any requirements that must be met to receive services and/or supports from the networks.
The Manager will attend network meetings, as needed, to ensure that all network participants are aware of the services provided by the CCSO.
Duties and Responsibilities:
- Provide community based and in-home services to FSP clients focused and should result in improving and/or maintaining the client’s ability to function in the family, school, or community
- Provide consultation regarding child development, behavior management, positive parenting and other issues of family life to natural parents, helping professionals, community organizations and others
- Develop close working relationships with HFS, DMH, and Value Options Collaborative, as well as other C&A providers throughout the north and west sides of Chicago, and residential facilities utilized by FSP
- Assess the client for indicators of abuse
- Establish a provisional DSM 5 diagnosis
- Facilitate psychiatric hospitalization when indicated
- As appropriate, provide brief and immediate mental health services or referral, linkage, and consultation with other mental health service providers
- Develop mental health assessment or admission note resulting in the identification of the consumer’s mental health services or referral, linkage, and consultation with other mental health services
- Provide case management, outreach and aftercare services
- Work cooperatively with other members of the crisis team, including co-workers and supervisors
- Maintain compliance with confidentiality policies and procedures at all times
- Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the community
- Assist with outcome measures
- Maintain compliance at all times with clinical documentation, timelines and agency policies and procedures including meeting required timeframes for documentation and service notes
- Report to assigned supervisor and actively seek consultation whenever necessary or requested by supervisor
- Attend clinical meetings and trainings
- Other duties as assigned by supervisor
The Community Relations Manager will:

directly interact with referral sources/other partners to develop connections among community options and the CCSO for enrolled youth

identify community options for wraparound staff to support the implementation of the plan of care for each enrolled youth

support care coordinators in accessing community networks and resources

assist community networks and resources in providing a welcoming atmosphere for families involved in wraparound

facilitate quarterly meetings with all identified service providers and networks that are willing to participate to discuss existing services and/or supports and areas for improvement in service availability.
The Community Relations Manager will submit a report detailing the strengths and areas for improvement discussing in these quarterly meetings bi-annually to HFS.
