ESSENTIAL FUNCTIONS:
• Contact referral source and client within 5 business days of receipt of referral.
• Carry a caseload of 9 slots per week. Part-time staff will conduct a prorated number of slots per week based on hours worked.
• Available a minimum of 3 evenings per week.
• Establish visit parameters with all parties involved.
• Provide transportation to family members who require it as per caseworker.
• Document activity and interaction between family members during visits and provide copies to caseworkers within 5 business days of visit.
• Complete program paperwork in a timely manner including end-of-month paperwork, weekly schedules, vehicle log/maintenance, expense forms, and evaluation forms.
• Complete all record keeping according to COA and DCF standards.
• Attend and participate in weekly team meetings. Attend all mandatory trainings.
• Participate in all DCF and YMCA meetings.
• Maintain a safe driving record.
• Report any abuse/concern of abuse immediately to a live person at DCP&P or police.
• Abide by proper car seat laws, rules and regulations.
• Insure assigned vehicle has routine maintenance, updated registration and insurance.
• Contribute time and effort towards a Strong Kids activity
• Demonstrate proficiency in the use of the Electronic Health Record.
a. Enhanced cleaning in areas where the employee is not directly/normally assigned as well as normally assigned areas. All employees are expected to assist in and support the efforts of the YGMC when it comes to health and wellness for the facility and other locations where the YGMC conducts business. All employees are expected to wipe machines, screens, computers, phones or anything that is used after they use it, all employees are expected to wash their hands on a frequent basis (recommended every 30 minutes if possible), and use hand sanitizer when hand washing is not available.

c. Directing Members/Visitors in areas where the employee is both normally and not normally assigned
d. The employee may be assigned to areas other than their normally assigned area as needed to meet organizational needs. Examples of this include, but are not limited to, being assigned areas to count people, help direct flow of traffic in an area, being assigned to a specific area to assist with doors, and any other duty as needed by the organization.
e. All other duties as assigned as it relates to COVID-19 or other communicable disease and/or pandemic.
