GENERAL DESCRIPTION
Responsible for Support Coordination for DDD clients according to DDD procedures. Enable DDD-eligible individuals to create stable, effective life-plans. Responsible for providing monthly follow-along services to each individual once the NJISP plan is approved by the Support Coordination Agency or the DDD-assigned Mentor.
EDUCATION / EXPERIENCE
Bachelor’s Degree. One year of experience related to individuals with IDD, social services counseling or related area.
JOB TASKS
- Provide support coordination services to DDD-eligible individuals according to DDD circulars. Advise individuals concerning DDD policies and procedures.
- Perform initial visit and obtain all documentation for new individuals. Scheduling contact to be made within 48 hours of receipt of the referral.
- Help individuals in preparation of all revised budgets and other program documentation.
- Ensure that all documents (Person Centered Planning Tool and New Jersey Individualized Service Plan) are prepared and submitted on a timely basis and correct upon submission.
- Research, visit, and build relationships with various providers in the community to inform families of community options in their area.
- Speak with individuals on a minimum monthly basis, and meet with individuals face to face on a minimum quarterly basis, minimum once in the home on an annual basis.
- Keep written records of all contacts with individuals in DDD EHR system.
- Keep documents and records up to date in internal CAU system.
- Ensure documentation is in CAU billing system by the 20th of the month in order to bill for services
- Maintain and clear alerts in all CAU and DDD systems
- Provide assistance in times of crisis or emergency as needed by the individual supported and help them reach the proper channels to obtain help needed.
- Complete Unusual Incident Reports within 24 hours of knowledge of an incident.
- Prepare weekly reports of all contacts conducted throughout the week
- Cooperate with any Agency, DDD, APS, and DCP&P investigation and inspection
- Ensures compliance with wearing a mask when mandated by State, Federal, and Licensing authorities during a public health crisis/pandemic.
