ESSENTIAL FUNCTIONS:
- Group counseling: plan, structure, facilitate pre-vocational groups maintaining focus on skill acquisition; monitors group attendance and individual progress and informs other counselors; identifies and obtains materials/supplies necessary to carry out group activities; identifies and accesses community resources to support group activities
- Supportive and Rehabilitation counseling: identifies consumer target/problem areas and supports problem solving and practical plans for resolution; helps consumers articulate their experiences; supports consumer strengths and resources affirming, reality-testing, contact with experience, judgment, and connection with family, friends and community resources. Helps consumer develop realistic service plan and collaborates to achieve measurable objectives.
- Service coordination: Refers and links consumers to Supported Employment; Facilitates coordination and communication among staff around pre-vocational needs; Involves/engages family and significant others in the recovery process and assists them in understanding the consumers illness coping with it and managing its effects. Consults with other service providers and documents their participation. Advocates on behalf of consumer and helps them do the same for themselves.
- Documentation: Prepares and maintains accurate up-to-date case records in accordance with agency standards, accreditation, and as regulated by DMHAS.
- Participates in Quality Improvement to assess and evaluate the effectiveness of pre-vocational services.
- Perform other duties as may be deemed necessary by the Program Supervisor and Director of Partial Care Services.
SUPERVISORY REQUIREMENTS: Provides supervision to consumers with co-occurring disorders in the vocational and food service program.
