Overview
The Coordinator of Young Adult Services facilitates the coordination and delivery of quality client services, offers clinical case management, and collaborates with external stakeholders to bolster young adult connections with outside agencies. Additionally, the Coordinator of Young Adult Services creates customized engagement plans, collects key outcome data, and serves as the primary source of support for guiding young adults experiencing homelessness through Breaktime’s three-year-long program model.

Roles & Responsibilities
- Coordinate services provided to young adults throughout the program model, including support accessing external services, creating customized engagement plans, and coordinating with external agencies serving young adults to ensure successful completion of the model
- Assess Associates' needs and support systems, and identify gaps in service provision to make appropriate service referrals to optimize Associate success
- Ensure quality and continuity of services to Breaktime young adults as they progress through the model
- Facilitate and evaluate progress towards achieving individualized goals with Associates
- Ensure diligent and thorough client data collection and provide metrics to drive optimized service delivery
- Participate in Program team meetings, All-Staff meetings, and Supervision
- Manage crises and assist associates in developing coping mechanisms
- Conduct case presentations
- Maintain accurate documentation of case notes
- Collaborate with Programs’ team leadership to develop, enforce, and refine Coaching policies and procedures
- Serve as an advocate for Breaktime Associates.
- Maintain communication with internal and external stakeholders
- Other duties as assigned or requested
Job Requirements
The Coordinator of Young Adult Services is an organized, detail-oriented, and skilled clinical case manager who thrives in a fast-paced and ever-evolving environment, and is excited to learn and support the needs of Breaktime young adults within, outside, and beyond Breaktime’s programming.
- Bachelor’s Degree in Social Work, Sociology, Psychology, Human Services, or related field with at least two years of experience working directly with youth, young adults, or individuals who are at risk for housing insecurity
- Enthusiasm for joining a fast-paced team environment
- Comfortable with technology and excited to learn/use new applications
- Extremely detail-oriented, organized, critical thinker, effective at time management, and working independently to manage tasks.
- Strong written and verbal communication skills
- Proficiency with Google Workspace
- A passion for working with and empowering young people
- (Preferred) Knowledge of, and experience with, data collection and analysis
- (Preferred) Experience using Salesforce
- (Preferred) Bilingual in Spanish, Portuguese, French, or Spanish Creole
