EMPLOYER INFORMATION:

Clifford Beers is a mental health clinic serving children and families. Our mission is to provide integrated services addressing mental, physical, and social determinants in order to improve health, resiliency and quality of life for children, families and communities.

PURPOSE:

This position is a vital member of the Integrated Care for Kids (InCK) system of care in New Haven providing resource mapping and oversight of a specific neighborhood based InCK Hub. Ensuring close collaboration amongst the neighborhood based partner agencies and resources. This position reports to the Director, Systems of Care.

Core knowledge that is fundamental to the InCK work in New Haven includes:

Extensive knowledge of the New Haven community including social services, neighborhoods, political landscape and community stakeholders

Flexibility to adapt to the changing/responsive nature of the New Haven InCK system of care.

The Integrated Care for Kids (InCK) Model, a federal grant awarded through CMS, is a child-centered local service delivery and state payment model that aims to reduce expenditures and improve the quality of care for children under 21 years of age and pregnant women covered by Medicaid through prevention, early identification, and treatment of behavioral and physical health needs.

KEY RESPONSIBILITIES:

Responsible for neighborhood specific engagement, including schools and healthcare providers (outreach, participation in existing events/activities; professional network contacts)
Outreach and engage members and caregivers on InCK services
Conduct all aspects of enrollment of an InCK member including administering a Needs Assessment
Single point of contact to assigned InCK providers, Intensive Care Coordinators (ICCs)
Monitor InCK provider and Intensive Care Coordinator (ICC) compliance to terms and conditions and provide contract
Facilitate regularly scheduled meetings with individual InCK providers to provide support, troubleshoot barriers to care, access and continuum of care needs.
Engage Care Coordinators for InCK, including training, best practices, assessment, referrals, technology platform and quality assurance
Facilitate quarterly meetings with assigned cohort of InCK providers and ICCs to address InCK system of care requirements, needs and best practices.
Generate monthly report of InCK activities and report back to the Director of Systems of Care
Oversee and track the training of all community partners in all workflows and technology
Manage continuous quality improvement: Monitor member and caregiver satisfaction
Update electronic platforms with current neighborhood resources and events (i.e. Community Calendar)
Organize and plan daily schedules
All other duties as assigned

Qualifications

EDUCATION AND EXPERIENCE:

Bachelor’s degree in MH/related field preferred or relevant lived experience
Community Health Leadership training a plus
Community Health Worker (CHW) certification a plus.

SKILLS:

Familiarity with New Haven community and New Haven area resources
Strong collaboration, organization and community engagement
Proven ability in complex problem-solving and critical thinking skills
Proficiency with technology, provider referral and member data systems
Proficiency in documentation, writing reports, aligning materials, proofreading and editing
Proficiency with project plans and deliverables, tracking and mapping activities and tasks
Agility and adaptability in response to changing priorities and timelines
Data proficiency in interpreting provider data and metrics to support positive outcomes and results

Valid driver’s license and reliable transportation

TRAVEL:

This role may require travel throughout the Greater New Haven area.
