JOB SUMMARY:
Coordinates pre-intakes, registration process, and case management linkage to appropriate services for the behavioral health programs of Catholic Charities, Diocese of Trenton.
DUTIES AND RESPONSIBILITIES:
- Receives and screens initial requests for services offering guidance with crisis navigation to internal and external parties as appropriate.
- Assuring department coverage Monday – Sunday 8am to 8pm
- Provides case management linkage for consumers to appropriate Catholic Charities services.
- Reviews psycho-social materials to determine appropriate level of care
- Knowledge of various insurances and ability to adequately explain benefit information
- Provide detailed description of agency programs
- Coordinate follow-up with referral sources
- Other duties as required
