Job Summary:
This position will provide primary health care educational and support services to SMI consumers who are enrolled in RIST.
Responsibilities:
Orient RIST consumers to basic principles of primary care management.
- Consult with RIST consumers and family members to develop and monitor wellness assessments.
- Assist in care coordination with the project RN to ensure access to primary health care.
- Help coordinate and ensure consumer access to the array of wellness and recovery education and self-help programs.
- Participate in regularly scheduled team meetings.
- Escort consumers to medical appointments and lab appointments, as necessary
