**Job Summary: **
We are seeking a Qualified Intellectual Disabilities Professional (QIDP)/Case Manager to join our team. The ideal candidate will be responsible for designing, implementing, and monitoring individual service plans for people supported with intellectual/developmental disabilities. As a Case Manager, you will coordinate services, advocate for people supported, and ensure the highest quality of care.
**Responsibilities: **
- Develop and review people supported service plans based on assessments
- Coordinate services and supports for people supported to ensure their needs are met
- Advocate for people supported rights and preferences
- Monitor and document people supported progress and outcomes
- Collaborate with caregivers, healthcare providers, and community resources
- Stay up to date on regulations and standards in the field
**Additional Information: **
- Flexible and hybrid work schedule.
- Most weekends off.
- No supervisory responsibilities.
- No on-call duties.
-Bachelor's degree in a Human Services field (social work, psychology, human services, etc.)
- QIDP certification or eligibility to obtain certification
- 1 year or more of experience working with people with intellectual/developmental disabilities
- Strong communication and interpersonal skills
- Ability to collaborate with multidisciplinary teams
- Knowledge of relevant regulations and best practices in the field
