Oversee daily operations and ensure all aspects of the residence run effectively 24/7, 365 days a year

Oversee case management activities, including psychological support, education about independent living skills and safety, facilitation of clients’ health and rehabilitation through direct services, advocacy, and referrals for mental health, medical, and substance abuse services, along with psychosocial, educational, and vocational services

Direct and supervise staff in health, mental health, and substance abuse activities, lead team meetings, conduct individual supervision, and provide exemplary leadership and program management

Coordinate referral and services with community providers, including Health Homes, inpatient, outpatient, emergency, day treatment, senior centers, and others, to ensure comprehensive access to services

Ensure compliance with multiple funding programs and audit requirements

Establish and maintain strong positive and productive working relationships, with an emphasis on positive and proactive problem-solving, with the staff of the program and other internal and external stakeholders and partners

Interface with HPD, HRA, and DOH via reporting, meetings, managing audits, etc.

Ensure accurate reporting and efficient and effective staff practices; develop and maintain policies, principles, and procedures that comply with ESSHI contract requirements, DOH, HPD, OMH, Medicaid, HRA, HUD, and other relevant state, city, and borough-based agencies, programs, and services

Ensure the program meets performance targets for housing stability, entitlement access, and reduced reliance on emergency health care services

Represent the programs and Comunilife in meetings as needed with regulatory agencies
-
In collaboration with the Property Management, ensure the facility, program operations, and all related documentation are in compliance with policies and procedures and requirements; liaise with internal and external stakeholders and partners as needed

Manage program funds and expenditures within the established budget, record and document expenditures in compliance with policies and procedures
-
Oversee incident reporting, utilization reviews, quality assurance activities, and audit preparation.

Promote recreational, social, and educational activities that reduce social isolation, improve quality of life, and foster independence
-
Review and investigate all client complaints and incident reports; engage in crisis interventions as needed
-
Orient and train new staff; manage staff performance through performance appraisals and ongoing supervision
-
Complete required weekly and/or monthly reporting accurately and on time
-
Identify and address service delivery gaps; develop linkages as needed
-
Monitor client lengths of stay and ensure regulatory compliance

Participate in the interviewing and selection of staff
