DescriptionJob Description: The Care Manager works as part of a comprehensive treatment team of professionals focused on the stabilization and care of individuals on our WellStone Emergencies Services - Pediatrics unit. Case Managers specifically support the team in identifying resources to assist youth and families during the treatment and after discharge. There are two full-time, salaried position available working Monday-Friday, 8am-4pm.
What you’ll be doing:
- Provide in-patient case management services.
- Provide direct services to clients experiencing a mental health crisis, to address needs that may have been unmet, as well as information about the continuum of care.
- Provide follow-up progress as per need of the client to treatment team members.
- Provide Basic Living Skills training when necessary.
- Assist clients with obtaining medication.
- Monitor medication compliance, maintain awareness of common side effects, and ensure the client is taking medication as directed.
- Provide traditional case management services and collateral services such as transportation to appointments, referral agencies, etc.
- Maintain high-quality client documentation and records management through electronic charting.
- Monitor the care and services clients receive in a residential setting.
- Develop and implement an Individual Service Plan which consists of goals, resources, and interventions.
- Revise client’s goals once met or progress as they occur.
- Assist clients experiencing mental health crises and arrange for such assistance from other professional or personal caregivers.
- Perform incidental care to families of the mentally ill and assist those family members in understanding the mental illness.
- Complete other duties as assigned.

QualificationsWhat we’re looking for:
- Bachelor's degree in social sciences field
- Excellent 5-year driving record & reliable transportation
- Excellent verbal and written communication skills
- Must be able to obtain the Alabama Department Mental Health (ADMH) Case Management Certification
- Knowledge of community lay-out (Madison County Area)
- Knowledge of community agencies and resources
