The IBHH Recovery Coach will be a part of the Integrated Health Home (IBHH) Team which is accountable for an individual’s total care and will deploy a multidisciplinary team-based approach to address the physical, behavioral, and social health needs for attributed individuals, utilizing a single Health Action Plan (HAP). Services delivered by the IBHH Team are highly individualized, address complex and chronic conditions, and capitalize on available health information and actionable data through the use of technology and appropriate information sharing methods. Services may be delivered via a mix of face-to-face and telephonic or electronic interventions but must ultimately be tailored in a manner that is unique to the individual’s needs so as to maximize engagement and activation.
Minimum Qualifications:
- Bachelors or master’s degree, no experience necessary.
- Associates degree plus two (2) years behavioral health work experience.
- High School Diploma or GED plus Peer Support Specialist certification plus three (3) years of behavioral health work experience.
- High School Diploma or GED plus Medical Assistant certification plus three (3) years of behavioral health work experience.
- High School Diploma or GED plus four (4) years of behavioral health work experience.
Preferred Education/Experience Qualifications:
- Bachelor’s or master’s degree in the Behavioral Health (BH)* related field.

Additional Requirements:
Must be 21 years of age (automobile insurance requirement) and possess a valid AZ driver’s license with a “clean” or “acceptable” driving record.
