Social Worker I - CADI/DD

WAGE RANGE: $53,000-$62,000/annually
JOB SUMMARY: Catholic Charities is seeking an initiative-takingindividual who is compassionate, trauma-informed, and culturally aware to work as a social worker in its CADI/DD program
This individual will need to work respectfully with children, adolescents, persons in poverty, and people of diverse racial and cultural backgrounds. They will be responsible for case management services, short-term counseling, and assisting clients with access to supportive services. They will help clients set goals for themselves and their families and providing ongoing aftercare to clients and their families.
They will additionally provide comprehensive counseling to clients of Catholic Charities per program needs and need to be able to collaborate effectively with clients with complex psychosocial concerns including mental health issues, unstable housing, unemployment, lack of medical insurance, dysfunctional relationships, and legal concerns. They may conduct support/education groups.
ESSENTIAL FUNCTIONS:
- Provide intake and comprehensive case management services including:
- Intake, interview, and assessment
- Goal setting
- Information and referral services
- Individualized client case management plan
- Provide assessment services and short-term counseling, skill building techniques, and aftercare to individuals, couples, and families.
- Provide client services in a variety of outreach sites in the community and division facilities as assigned. Conduct support/education groups as needed.
- Responsible for reviewing and ensuring the relevance, accuracy, and timeliness of data and reporting for purposes of accountability and client outcomes.
- May provide behavioral management support and consultation to collateral sources and coordinate joint efforts to address needs or problems of the client population being served.
- Make appropriate referrals and provide comprehensive decision-making counseling to help ensure client’s needs are being met.
- Develop cooperative working relationships with communities including schools, parishes, and other professionals on behalf of the clients we serve. Work collaboratively with outside agencies to provide behavior management support and consultation.
- Other duties as assigned.
MINIMUM QUALIFICATIONS/EXPERTISE:
- Current Mental Health Practitioner licensure approved by the State of Minnesota required (e.g., LSW, LGSW).
- One (1) year of broad-based clinical experience preferred.
- Experience in individual, family, and group counseling preferred.
- Ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness required.
- CPR/First Aid Certification required.
