Position Summary
The Justice Housing Coordinator at Lutheran Metropolitan Ministry (LMM) is tasked with delivering housing navigation and case management services to justice-involved individuals facing homelessness or housing instability. This role operates within Housing & Shelter Program’s (H&S) Housing Team and focuses on supporting individuals at the Men’s Shelter, those transitioning from incarceration, and participants in LMM’s Workforce Development programs. The primary objective is to facilitate housing stability by guiding clients towards permanent housing, enhancing income, connecting them to behavioral health and medical services, and providing comprehensive housing navigation. The principal goal is to make homelessness a brief, rare, and one-time experience, actively preventing homelessness through the delivery of high-quality, responsive housing navigation, and case management services for justice-involved individuals.
Some of the duties include:
- Engage and provide case management and housing navigation services to justice-involved individuals experiencing homelessness and/or housing instability
- Support clients in developing housing plans for a rapid exit from homelessness or safe housing post-incarceration, ensuring linkage to suitable resources
- Connect clients with community partners for essential services, including identification, benefits, mental health, substance use disorder support, and appropriate housing interventions
- Assist clients in increasing income through linkage to job training programs, employment opportunities, or SSI/SSDI applications
- Ensure linkage to Cuyahoga County’s Coordinated Entry, if experiencing homelessness and not assessed prior to Shelter entry
- Arrange and/or provide transportation to critical destinations such as housing, medical appointments, and employment
- Maintain accurate participant data, statistical information, and client records in LMM’s internal database (HOPE) and Microsoft Excel tracking systems
- Establish and maintain regular communication with appropriate partner agency staff for individual client/staffing updates
- Collaborate with H&S’s Clinical Team on mental health and substance use crises
- Participate in regular caseload reviews with supervisor(s) to track progress
- Conduct crisis interventions for clients, as needed
Qualifications
Our ideal candidate has a High School Diploma or GED; Associate’s/Bachelor’s Degree preferred. Some experience working with people experiencing homelessness, veterans, people with substance use disorders, people with re-entry backgrounds, and/or other relevant social service populations, preferred. A Valid Ohio Driver’s license and proof of insurance, required.
