Description

POSITION SUMMARY:

Residing in ECSs Interim Housing Department, the temporary Shelter-in-Place (SIP) hotels are a short-term solution for temporary housing in response to the COVID-19 public health crisis. Each hotel will have a Care Coordinator, who reporting into the Associate Director of Interim Housing, will oversee all care coordination for participants from intake to exit planning.

PRIMARY DUTIES AND RESPONSIBILITIES OF THE PROGRAM MANAGER:

Oversee intake process, including completion of participant files, ensuring all required documentation is signed and filed.
Assist in the stabilization of participants from pervious homeless setting to SIP Hotel, including connecting and coordinating with IHSS, nursing staff, or other services as needed.
Assist in the monitoring and reporting of data collection procedures associated with ECS needs and as required for City contracts in participants files and databases.
Work with the Covid Command Center (CCC)/HSH to communicate exit/housing plans to participants and communicate status updates in One System.
Lead and/or assist with weekly care coordination/case conferencing meetings regarding participants needs, ensuring behavioral health referrals are requested and properly documented as appropriate.
Setting up, notifying, and reminding participants of upcoming appointments for health, housing, benefits, and other necessary appointments.
Coordinating with outside providers, agencies, and departments to provide direct services to participants staying at SIP Hotel.
Together with CCC and HSH, identify barriers and advocate for system change and document progress for clients to receive financial and other benefits.
Work with participants and other partner agencies to have guests exit into appropriate housing, treatment, or other stable placement and providing a smooth transition.
Follow and implement all policies and procedures of ECS and SIP hotels.
Establish and maintain a positive, collaborative working relationship with employees, volunteers, and participants of The SIP Hotels, and with other partner agencies.
Attend all required organizational meetings, community meetings and professional development trainings.
Perform other duties as assigned.

Qualifications

REQUIREMENTS:

Bachelors Degree in Social Work, Psychology, Counseling or a related field, and at least two years paid work experience with adults experiencing homelessness and/or multiple disabilities; or an Associates Degree with five years paid related work experience.
Must have strong computer skills.
Possess a good working knowledge of and a sensitivity to individuals with substance abuse and/or mental health issues such as: PTSD, psychosis, borderline behaviors, bi-polar, etc.
Experience working with individuals in crisis or unstable housing situations.
Demonstrated respect for, experience with and willingness to work with a diverse workforce and participant population.
Professional and discreet demeanor with good interpersonal skills.
Ability to handle confidential information and use of tact, respect, discretion and judgement.
Experience working in integrated service teams using participant-centered, trauma-informed, harm-reduction approaches.
Commitment to ECSs mission and to its organizational values.
Knowledge of community resources and methods of accessibility.
Strong interpersonal skills, a team approach, and the ability to communicate both orally and in writing are essential.
The ability to quickly assess and respond to a variety of situations, including medical emergencies, conflict resolution and intervention, and other participant emergencies.
Ability to be flexible and adapt to changes as this program responds to lessons learned and emerging needs.

OTHER:

Must secure finger image screening and annual TB screening. Must be able to meet physical requirements of the position which may include standing, sitting, walking, reaching, bending, twisting, pulling and lifting up to 30 pounds.
