Signe Burckhardt Manor and Thomas Feeney Manor are hiring a Full-Time Resident Care Coordinator to join the team! This joint role would spend half of their time at Signe Burckhardt Manor and half of their time at Thomas Feeney Manor.
In this role, you will advocate for our residents in long-term care and family members to ensure they receive high-quality care and services.
This is a Full-Time, benefit eligible position work 40 hours a week (20 hours at Signe Burckhardt Manor and 20 hours at Thomas Feeney Manor).
Pay Range: $26 / hour

Resident Care Coordinator responsibilities:
- Provide valuable social services to residents and their families in medical, emotional, physical, or psychological concerns.
- Assist the team in coordinating social services for the facility, individualized plans for the residents, medical equipment, counseling or other follow-up treatment plans.
- Assist residents with their care coordination needs and provide one on one services.
- Commitment to compassion, excellence and quality care.
- Act as liaison between families, staff, and various agencies.
- Act as an advocate ensuring basic human and resident rights.
- Conduct and coordinate Admission, provide appropriate referrals and discharge planning of residents.
- Provides resources and referrals to staff, residents, and family members.
Resident Care Coordinator skills and requirements:
- Transitional or long-term care experience preferred with an opportunity for advancement.
- Must have knowledge of state and federal guidelines related to long-term care and resident rights.
- Successful candidates will be mission-minded with excellent communication skills.
