CCH PROGRAM DIRECTOR- COMMUNITY CAMPANION HOME
SUMMARY
This position is responsible for the coordination of services for individuals supported in our Community Care Homes/CCH Program. The CCH Program Director will be familiar with and carry out support, training, and licensing process in adherence with CT Department of Developmental Services (DDS) regulation and guidelines. In addition, this position will support the recruitment and on-boarding of new licensees, with the purpose of expanding services to individuals in need of CCH services. Our Program Director of CCH will manage our CCH Coordinator directly.
ESSENTIAL FUNCTIONS
- Manager one direct report (CCH Coordinator)
- Active case management to identify and support the needs of the individuals and families
- Maintenance of licensing requirements for all CCH homes per state regulations
- Recruitment for and Development of CCH program
- Development, evaluation, and implementation of training for all licensees per state and agency regulations
- Respectful and supportive engagement with individuals and families
- Ongoing communication and rapport-building with DDS Case Managers
- Familiarity with and implementation of agency priorities
- Active participation in departmental and agency-level meetings and events
RESPONSIBILITIES
- Makes monthly visits to CCH providers/individual residents and interfaces with DDS Case Mangers to coordinate care and support.
- Completes home study evaluations and recertifications and attends all licensing reviews as required by DDS.
- Schedules and facilitates individual care planning meetings (ISP) and writes quarterly/annual reports as required.
- Makes referrals for and coordinates services for supported individuals as needed and communicates effectively with all parties involved.
- Coordinates application process for CCH programs, matching referrals to licensee homes based on the preferences of both parties.
- Ongoing collaboration with Aspire Adult Services Team and various departments within the agency to stay current with and contribute to agency priorities.
- Responds to internal and external audits in a timely fashion and actively engages in quality improvement processes.
MINIMUM REQUIREMENTS
- Bachelor’s degree in human services, social work, psychology, counseling, or a related field.
- Strong interpersonal, organizational, and communication skills.
- Strong computer skills
- Valid driver’s license and safe driving record.
- Reliable transportation and willingness to travel to sites throughout CT.
- 18 years or older.
