As the System of Care Coordinator, you will work with a small caseload of families with the primary focus on the child struggling with mental health/behavioral issues which could place the them at risk of removal from the home. Using the High Fidelity Wraparound model, the System of Care Coordinator works with the family intensively, to build rapport and move them toward better overall functioning. Strong candidates will have prior experience using an electronic medical record/billing system. There is a very manageable productivity expectation.

Program Statement: Our System of Care program provides care coordination services for individuals and families referred by the Division of Children and Family Services, and/or the Juvenile Court in Cuyahoga County. Services will be designed using the wraparound model of service planning.
What you will do:
- Develop Plans of Care and Crisis plans for families.
- Work with family teams to identify family strengths and needs in order to access needed services.
- Act as an advocate for program participants with courts, schools, treatment centers, and other services as appropriate in order to make referrals and conduct follow-ups.
- Provide therapeutic behavioral services or community supportive psychiatric treatment to bill Medicaid. (based on licensure)
- Work closely with staff from neighborhood collaborative partners to ensure coordination of services and to avoid duplication.
- Prepare written reports, regular case notes and summaries for consultation according to program standards and professional ethics.
- Maintain all documentation required for Medicaid billing and other program specific documentation.
- Perform other duties necessary to accomplish objectives.
