Position Summary:
The Supervisor is responsible for managing the team and supervising the LifeSet Case Managers in their day-to-day implementation of LifeSet, a model to address the needs of young people who are transitioning out of care and need individualized skill development and support to transition successfully into adulthood, specifically obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills, obtaining and maintaining employment, building a support system, and remaining free from legal involvement. The partnership with Youth Villages offers an evidence-informed program model to serve young people 17-22.
Responsibilities:
Provide supervision to the LifeSet Specialist Case Managers
- Assist Program Director with the hiring of the Specialists
- Carry out supervisory responsibilities in accordance with Catholic Charities policies
- Manage the professional development of each Specialist by completing a development plan and conducting individual supervision meetings and completing annual performance reviews
- Work with the LifeSet Clinical Consultants to provide direction to the Specialists based on the therapeutic model which includes weekly case reviews and facilitation of the group supervision process
Oversee the work of LifeSet and adhere to model fidelity and best practices
- Access on-line portal with evidenced based and best practices that inform case consultation and service plan development and review
- Oversee service plan development and ensure case progress
- Support staff when attending court and meetings and conduct field supervision to ensure Specialist competency
- Ensure documentation and contractual requirements are met
- Assist with planning, assigning and directing work and collaborating with internal and external customers
Participate in professional development, consultation and supervision
- Attend weekly consultation led by a licensed professional
- Participate in weekly supervision with the Program Director to provide support on professional, operational and clinical goals
- Consult with Youth Villages Regional Lead as needed
Provide an on-call 24-7 rotation with the Specialists, offering in person support if needed
- Assist with coverage when needed when Specialists are on vacation or sick leave
Perform other duties as assigned

- Masters degree in field of Social Work, Psychology or related human services field with experience preferred
