- This shift is 40 hours per week (Mon-Fri 9am-5pm) some flexibility needed
- The pay rate for this position is $23.07 hourly
Qualifications:
- Bachelor’s degree in a related field
- Two year's supervisory experience in the field preferred, but not required
- Excellent verbal and written communication skills
- Computer literacy and a willingness to learn and utilize new electronic systems
- Valid Driver’s License required and car for travel
- Ability to work a flexible schedule
- Must have experience with OPWDD
Job Duties:
- Assist in the development, implementation and monitoring of valued outcome goals with Individuals and Community Habilitation Support Staff
- Actively coach Community Habilitation staff to support ongoing regulation changes
- Ensure that Valued Outcome Plans are person centered
- Ensure that Community Habilitation support staff is accurately documenting Community Habilitation Plan work with Individuals via both paper and electronic formats
- Maintain site control as necessary, providing at least one home visit semi-annually for each Individual/advocate with staff present
- Conduct ongoing phone calls and/or at least one spot check visit annually during their Community Habilitation Support staff’s work shift
- Ensure that Staff Action Plans Are reviewed in conjunction with six-month reviews and annual Life Plan meetings or on a more frequent basis if needed
- Participate in ongoing training and supervision to adapt to program changes
- Maintain and update all the Community Habilitation documents according to OPWDD regulations
- Assist Individuals and staff with planning, coordinating and attending person-centered activities within the community
- Work at multiple sites on an as needed basis and as required by supervisor or family, particularly for recreation and emergencies needs
- Be able to enroll new Community Habilitation support staff and Individuals into the program
- Complete payroll and billing for Community Habilitation Staff & Individuals utilizing paper and electronic formats
- Ensure Individuals’ documents are audit ready at all times
- Willingness to provide services to individuals on an as needed basis as required by departmental needs
- Work in conjunction with Admissions to enroll new individuals into the program
- Work in conjunction with Recruitment and HR to onboard and new hires
- Complete payroll and billing for Community Habilitation Staff & individuals, utilizing paper and electronic formats
- Ensure Individuals' documents are audit ready at all times
