Position Summary:
Provide support to Easterseals families who have been referred by Early Steps Florida for early intervention services. Serves as a family advocate, and liaison between family/client and community services and a support system for families within the child development center, and the community in accordance with program requirements.
Duties and Responsibilities
Provide primary service coordination services to children and families enrolled in Easterseals Civic Center and Miami Gardens Early Steps/MDCPS Birth -Two program.
Provide information, referral and follow-up within the contract requirements.
Participate in multi-disciplinary meetings and evaluations.
Coordinate and participate in all Early Steps Individual Family Support meetings and support family in transition to other pre-k programs upon child's third birthday.
Serve as point of contact and liaison with Miami Dade County Public Schools Pre Kindergarten Program for Children with Disabilities.
Ensures compliance with Eligibility, Enrollment and Attendance criteria as it relates to Early Steps and Miami Dade County Public School Birth-Two Program
Provide support to Easterseals Family Engagement staff during child/family enrollment to ensure that Early Steps/Miami Dade County Public School Birth-Two Program eligibility and program requirements are explained to family.
Consult and follow up with other professionals (i.e. physicians, therapists, and/or Infant Toddler Developmental Specialists) involved in the treatment of the children.
Maintain necessary records and reports; submit documentation in accordance with agency and contract requirements.
Build and maintain effective relationships with internal and external partners.
Attend in-service training, supervision and meetings as required.
Performs all other duties as assigned by supervisor.
Other:
Ensure the safety of all staff, volunteers, visitors, and individuals receiving services at Easter Seals.
Act in compliance with Medical and Non-Medical Emergency Procedures.
Assist individuals in evacuating buildings during emergency situations to include but not limited to: fire drills and external disaster drills. Specific duties to be assigned by immediate supervisor.
Assume assigned responsibility for non-clinical duties during disaster and/or drills including but not limited to safety of files/office areas, closure of doors.
Successful completion of background process which includes Local background check, Level 2 clearance (FBI, FDLE), clear TB test, medical clearance, drug test and reference checks.
Work a flexible schedule as required.
Other related duties as necessary to ensure the smooth running and efficiency of Easter Seals South Florida as a whole.
