JOB SUMMARY: Provides, within agency objectives and practices and under the supervision of the Clinical Director, individual and group early intervention education services to individuals and their families as well as assists the Program Director in designated administrative tasks related to reporting and data collection to state entities and internal reporting entities.
As a member of a team, the Prevention First Program Coordinator provides leadership and management skills to the Prevention First Services program. In addition to the responsibilities of the Program Coordinator, he/she also maintains the responsibilities of the Prevention Specialist, promotes the services to the community, develops programing, develops program schedule, and collaborates with other departments within Catholic Charities.
ESSENTIAL FUNCTIONS:
Clinical:
- Conduct educational services to service schools and prepares lesson plans.
- Utilizes structured curricula and protocols to promote psycho-education and prevention skills, skill acquisition and to maintain focus in groups
- Required to carry a caseload, facilitate groups, and educational sessions.
- Develops, with clients, realistic lesson plans based on target/problem areas, and collaborates to achieve measurable objectives
- Meets individually with educational facility leadership on a weekly basis to monitor progress towards objectives identified in lesson plans and to assure schedule meets needs of program.
- Involves and engages family and significant others in the education process and assists them in understanding the program
- Arranges treatment meetings with state auditor and site lesson reviews
- Provides program leadership
Service Coordination and Documentation:
- Ensure client confidentiality by following strict rules and regulations of agency policies
- Establishes and maintains communication with referral sources, other authorized agencies, and treatment providers
- Keeps track of lesson plans and data collection of services.
- Identifies and links community resources to support group activities
- Assist staff in developing curriculum schedule and assures that they meet grant requirements
- Maintain accurate and up-to-date program records/files consistent with agency and funding requirements
Program Coordinator Responsibilities:
- Oversees the development of the clinical and educational prevention component of the program
- Maintain established program productivity standards and monitor staff productivity standard achievement
- Monitor budget to grant capacity as not to exceed budget allowance
- Assures staff meet training needs of the program
- Responsible for facilitating weekly supervision with prevention specialist/staff
- Conducts weekly data collection and documentation review and assures they are complete and accurate
- Facilitates Multidisciplinary Team Meetings to maintain focus of program goals
- Attend all required trainings and stay current with required online trainings
- Adheres to codes of conduct governing family/family and therapist/staff relationship as established through Catholic Charities, the National Association of Social Workers, the American Psychological Association, the National Association of Marriage and Family Counselors, or other applicable codes of conduct
