PRIMARY RESPONSIBILTIES:
The primary function of the Program Specialist is to provide coordination services to consumers/families being admitted into MYPAC and Wraparound service programs. Community outreach and fostering the program’s relationships with other agencies are essential duties as well.
ESSENTIAL DUTIES:
Assist with crisis call line and rotation.
Assist consumers/families with program enrollment.
Adheres to a flexible schedule to meet the needs and schedules of clients. Provides follow up/aftercare discharge services as appropriate.
Participates in team staffing.
Coordinates with staff to work cooperatively and efficiently to meet consumer needs.
Assist with program satisfaction survey and discharge questionnaires to include distributed, collection, and entry into data base.
Serve as advocate and mediator between referral sources, families, and funders
Monitor insurance eligibility status of consumers.
Completes data entry into WITs database for Home Base Program. Assist with the scheduling of psychiatric appointments as needed. Provides Case Management duties as needed.
All other duties as assigned by the Community Services Program Director.
KNOWLEDGE/EXPERIENCE:
Minimum of Bachelor of Social Work or related degree; LSW, or equivalent. Must have at least two (2) years of experience working with children and families and at-risk populations. Must be knowledgeable of trauma- informed care and local community resources.
SKILLS / ABILITIES:
Excellent time management, organizational and stress management skills.
Must be able to maintain confidentiality of program and agency information.
Must be able to follow written and verbal instructions and communicate effectively. Proven admin or assistant experience.
Knowledge of office management systems and procedures. Attention to detail and problem-solving skills.
Excellent written and verbal communication skills. Strong organizational and planning skills.
Maintains a high level of professionalism, integrity, respect for others, and a positive attitude always. Must be able to problem solve and apply conflict resolution skills when needed or as directed by the
Community Service Program Director.
Proficient in MS Office.
