For more than 30 years, SCCS has provided care, counseling, coaching, and essential mental health services for children, teen, young adults, and their families. The Bilingual Clinical Assessor provides therapeutic and prevention services to clients and families via a combination of in the office located in Colton and telehealth; includes case management, education, de-stigmatization of mental health issues, and crisis intervention. Demonstrate high legal and ethical standards and operate from a culturally competent and strengths based approach.
The Clinical Assessor will fulfill productivity requirements, comply with all documentation standards, and participate in an on-call rotation. This position will be responsible for facilitating small support and psychoeducational groups. If necessary, the Clinical Assessor is responsible for coordinating Mental Health services and Link and Consultation services in a manner to ensure follow-through on court ordered service plan, IEP Mental Health Plans, or development of a plan for families that are referred.
The Clinical Assessor will also coordinate the overall care and transition of clients within our programs or to a community partner should it be necessary and serves as a support and case manager to provide screening and assist in linking services needed by members served and their family and by centralizing this coordination, we are able to eliminate the risk of clients being overlooked, experiencing unnecessary delays, or receiving inadequate services. The Clinical Assessor Client Care Coordinator must be able to offer a level of sophisticated centralized oversight between programs, other SCCS teams, and outside agencies.
JOB DUTIES & RESPONSIBILITIES:
- Track clients with whatever-is-needed-communication to make sure there is a secure “hand-off” between programs, when clients transition from one program to another, whether within SCCS or not. Because this is a type of non-billable case management service it will not be done by a clinician, but by the Client Care Coordinator, demonstrating our dedication to being good fiscal stewards of County funds.
- During the referral process, identifying clients with recent crisis events and prompting linkages to higher level care.
- Conduct psycho-educational related to stress management, self-care, mental health, and/or substance to minimize and prevent crisis related incidents
- Provides in person and / or remote response to crisis situations in accordance with program guidelines
- Coordinates individual’s care with identified Support Coordinator and applicable crisis responders to include service linkages
- Process referral requests from the different departments of the County of San Bernardino, unified school districts and self-referrals.
- Identify appropriate SCCS programs based on triage results during the referral process.
- Identifying clients at risk of losing their MediCal status and prompting assistance to ensure family maintains active MediCal whenever possible.
- Perform data entry into EHR and update information as needed.
- Answer phones and direct calls to the appropriate persons or services.
- Answer phone call and email of prospective patients and assist in coordinating care in a polite and timely manner.
- Provides individual psychotherapy, group therapy, and family therapy as determined during Plan
- Development session and or by the Child and Family Teams and reflected in the Family’s Plan in the family’s home and/or community
- Appropriately manage crisis situations.
- Accurate, thorough, complete, legible and time specific documentation on a daily basis, i.e., within one business day and within the SCCS documentation quality standards.
- Consult with agency psychiatrist, school psychologists, teachers and other professionals in the community to ensure continuity of care.
- Maintain trainee/intern/licensure status with BBS.
- Promptness in contacting new clients, prioritizing schedules of clients based on client and agency needs as well as the timely closing of cases.
- Participate in outcomes measurement tools administration and collection.
- Complete accurate and professional assessment and diagnosis of mental disorders.
- Constant professional communication and collaboration with other staff.
- Participates in on-call rotation and follows procedures regarding on-call.
- If necessary, outreach in community, consisting of presentations to groups of all sizes and participating in outreach activities/resource fairs.
- Able to facilitate psychoeducational and support groups by using Evidence Based Practice interventions and curriculum, as well as prepare flyers and other educational materials.
Clinical Assessor
Role Summary: To provide full psychosocial assessment including, diagnosis, identifying impairments, and determining medical necessity/program criteria are met.
- Population: All ages. Assessments are completed for the following programs: SAP, MHSSA, SATS, Success First, CWE, and GMH.
- Caseload & Productivity Expectations: Complete 2 assessments per day
- Work Schedule: Monday-Friday 8:30 -5pm with one late day of 10:30-7pm. Some flexibility upon supervisor approval.
- Services Provided: Assessment, crisis intervention, treatment planning.
