As a member of the interdisciplinary team theLPN/RN will be responsible for the following:
Responsible for completing all Ambulatory Detox/MAT admission required assessments including, nursing assessments and phlebotomy
Monitors and tracks consumers vital signs and indicators of withdrawal using the COWS scale
Maintains accurate documentation in the MAR in accordance with agency policy and take home medication policy
Collaborates with medical providers and scheduling appointments, using electronic health record (Anasazi) scheduler
Implements medical interventions as prescribed by the physician
Coordinates processing of laboratory specimens and review of findings and collaborates with lab providers
Completes program specific required assessments and documentation (CCBHC documents), including accurate documentation of PF, AD, MAT service codes into multi-service progress notes
Consult and provide information to program staff regarding consumer health and/or medication issues as well as the results or changes made to a clients medication by Physician as appropriate
Input consumers community medications into the Anasazi system and keep up to date.
Complete any pre-authorizations needed for consumers medications.
Communicate/follow up with Pharmacy as needed
Administration of oral medications and injections routinely ordered for consumers
Conduct 1 nursing group weekly and complete multiservice group progress notes
Participate in interdisciplinary team meetings with other Project Free staff or outside facility staff.
Attend weekly PF staff meetings
Review and sign consumer IRPs
Provides psycho-educational services and support
Provides after-hour emergency on call telephone coverage to consumers and their families on a rotational basis.
