The Program Manager will provide administrative and managerial guidance to the Healthy Moms Healthy Babies program with strong planning, directing, analyzing, and organizational skills. This role utilizes a strengths-based approach and focuses on empowering each client while completing home visits.
Healthy Moms, Healthy Babies is a comprehensive case management program that offers pregnancy and parenting support to expectant mothers and families. Our case managers provide pre- and post-natal education with the goal of improving pregnancy outcomes. Our clients learn new skills for a strong, self-sufficient lifestyle. This includes a better understanding of infant growth and development and improved family economic foundation.
What you’ll do:
- Provide comprehensive case management to clients for the purpose of self-sufficiency.
- Attending community networking meetings by representing the agency.
- Create and update individualized care plans for clients per the requirements and needs of the individual.
- Conduct the enrollment process in the program with clients
- Provide case management with each client in combination with phone and face-to-face visits in varied settings, including the client’s home.
- Complete written and data-based documentation (i.e., internal documentation case notes, etc.)
- Assess and monitor clients' parenting skills with appropriate intervention, counseling, education, and support. Education will focus on prenatal and postnatal care.
- Performs other related duties as assigned.
All you need for success:
Minimum Qualifications
- Bachelor’s degree in social work (BSW) or equivalent human service degree.
- This position requires a valid driver’s license, a reliable vehicle, and proof of insurance.
Preferred:
- Master’s degree in a related field.
- Minimum two years of experience in the social service field.
- Supervisory experience.
