GENERAL DESCRIPTION
Handle all calls for information and referral regarding CAU services and programs. Develop individual contacts with referral sources. Provide referral information to consumers if CAU programs are not viable. Responsible for implementation of program policies. Coordinate intake process from initial referral to admission into CAU services.
EDUCATION / EXPERIENCE
Bachelor’s degree in Social Services or related field. Minimum of two years experience with a Social Service Agency.
JOB TASKS
- Maintain in-depth knowledge of all Community Access Unlimited programs and services
- Enhance the current level of contractual services within our departments
- Increase public relations and community education effectiveness.
- Coordinate the Agency’s intake process timely and effectively.
- Coordinate, track and process all possible referrals
- Coordinate site visits for family’s interested in services.
- Research and participate in various conferences, seminars, and community fairs and other events to showcase CAU services.
- Assist in the creation and maintenance of marketing materials that showcase CAU services.
- Maintain relationship with various entities to ensure LOS for agency is met by receiving referrals.
- Promote the Community Support Program for inquirers who do not have state aid or who can not access other services.
- Offer Community Support Services to all Family Access families.
- Promote educational and recreational opportunities.
- Maintain a comprehensive data base of community services and resources and disseminate information and make referrals as appropriate.
- Primary contact between CAU, Support Coordination agencies and DHS.
- Assist in the Agency’s public relations efforts and community education as necessary
- Report monthly on vacancies and admissions into CAU services
- Cooperate with Agency, legal authorities and Department of Human Services or any part or successor thereof in any inspection or investigation
- Actively solicit new member referrals and harvest new service contracts for CAU
- Ensures compliance with wearing a mask when mandated by State, Federal, and Licensing authorities during a public health crisis/pandemic.
- All other duties as assigned
SHIFT
Typically Monday – Friday, 9 a.m. – 5 p.m. Some late nights and weekends
